Vaginal agenesis, has a prevalence of 1 in 4000 females and it is mainly found as a feature of the Mayer-Rokitansky-Küster-Hauser syndrome. Primary amenorrhea during adolescence is the prominent symptom. In addition diagnosis is confirmed by the ultrasonography, the gynecological examination under anesthesia and the laparoscopy. An intravenous pyelography is also suggested to exclude urogenital anomalies, which coincide in 40% of the cases. The management of the patients, along with the proper psychological support and reassurance, includes the creation of a neovagina during adolescence to offer them the opportunity for a normal sexual life. Several non surgical and surgical techniques have been proposed for the creation of a neovagina as the Frank method and its modification by Ingram, the McIndoe operation, the Vechietti procedure and its laparoscopic modification, as well as the Williams vaginoplasty and its Creatsas modification. Creatsas vaginoplasty is a simple, fast and safe technique that has been applied in 254 cases over the last 20 years providing excellent results in the vast majority of the patients. The technique (video presentation) last no more than 20 minutes and the mean hospitality stay is approximately 4 days. The relative results (case reports) are included in this presentation.
