Evaluation of the use versus non use of urinary catheterization during caesarean delivery.

Objectives: To evaluate the necessity of routine indwelling of urinary catheterization during caesarean delivery. Materials and Methods: This was a prospective comparative study was done in OBGYN dept from 1st August 2012 to 30th June 2013 on 272 (136 c/s with catheter and 136 c/s without catheter) women who underwent caesarean section. Outcome depends by comparison between 2 groups on operation time, difficulty in extraction on fetus, bladder injury, I/v fluid during operation, time till first voiding, time till ambulation, postoperative burning maturation, frequency, haematuria, postoperative urinary retention, PPH, postoperative UTI, Post operative hospital stay. Result: Operation time was similar statistically between CS without catheterization group and CS with catheterization group (37.28 ±7.74 minutes vs 36.47 ±7.43 minutes; t=0.879; p=0.380).Time till first voiding [5.65 ±1.22 hours vs 12.93 ±1.37 hours; t=-109.006; p<0.001]; time till ambulation (hours) [14.16 ±2.31 vs 20.51 ± 3.16; t=-18.909; p<0.001]; were significantly earlier in CS without catheterization group than that CS with catheterization group. Burning micturition [14 (10.3%) vs 67 (49.3%); χ2=49.386; p<0.001], frequency [9 (6.6%) vs 46 (33.8%); χ2=31.200; p<0.001] and haematuria [4 (2.9%) vs 19 (14.0%); χ2=10.686; p=0.001] were significantly fewer in the CS without catheterization group as compared to CS with catheterization group. Development of UTI was significantly higher in the CS with catheterization group as compared to CS without catheterization group (χ2=31.200; p=0.006). . The length of postoperative hospital stay significantly more in patients of CS with catheterization than that of CS without catheterization (t=-2.003; p=0.046). Conclusion: Routine urinary catheterization during cesarean delivery in hemodynamically stable women is unwarranted.
